GENERAL SUMMARY: Under general supervision, provides and coordinates the care provided to transplant patients in the pre-transplant phase of care. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of professional practice in the delivery of patient care and in participation of the development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness. Is accountable to patients and families for the processes and outcomes of care during the pre-transplant phase. Is accountable to peers for the advancement of group practice within the Department of Transplantation. Responsibilities:
  • Provides comprehensive patient care to and coordinates multidisciplinary care for solid organ pre-transplant candidates.
  • Provides outpatient follow up pre transplant, interacting with all disciplines as necessary.
  • Provides direct support to physicians and patients 24 hours a day as organs become available for transplantation.
  • Coordinates data collection for transplant patients.
  • Maintains transplant waiting list, including direct contact with Organ Procurement Organization.
  • Assists financial counselor, nutritionist, pharmacist, and social worker with patient issues/information as necessary.
  • Provides ongoing patient/family education and support.
  • Collaborates with team members at transplant patient care meetings by preparing data and maintaining a record of proceedings.
  • Develops and maintains accuracy of all transplant related protocols.
  • Assists the Medical Director in a variety of areas to facilitate the Transplant Program.
  • Assists with promotion of the transplant program through interaction with referring physicians, other medical institutions and education of dialysis unit staff.

    What You Need to Be Successful:

    • Licensure as a Registered Nurse in the State of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.
    • BSN preferred.
    • Dialysis, Critical Care, or Transplant experience is preferred.
    • Must obtain NATCO certification within 2 years of hire, or within a timeframe determined by management. Must maintain certification once NATCO certified.
