Strategies to Assess and Meet Patient Care Needs

A Collaborative approach is utilized in caring for our patients. Clinical assessment, patient interviews, rounds, patient outcome, and satisfaction data and communication, determine the patient's care requirements in the CCW with the patient and/or their designation spokesperson.

A multidisciplinary team delivers care to patients in the CCW, which includes intensivists, attending physicians, fellows, house staff, nurse practitioners, nurses, patient care technicians, unit clerks, social workers, physical/occupational therapists, pharmacists, dieticians, and other ancillary services.

All Services document in the medical record, which supports multidisciplinary patient care delivery along with daily patient rounds. Patient education is an interdisciplinary process. The patient's individual plan of care includes an educational assessment with educational interventions matched to patients and family educational needs.
